Responsibilities

The Social Worker provides services to respond to the emotional needs of residents and their families. 

  • As a valued member of the healthcare team, the Social Worker completes admissions, social services evaluations, provides for social services or referrals as needed, and maintains communication with resident and families.  
  • Provides medical related services so that each resident may attain or maintain the highest practical level of mental, physical and psychological well-being.
  • Makes referrals and assists with obtaining services from outside entities (e.g., talking books, absentee ballots, community wheelchair transportation);
  • Assists residents with financial and legal matters (e.g., applying for pensions, referrals to lawyers, referrals to funeral homes for preplanning arrangements);
  • Provides Discharge planning services (e.g., helping to place a resident on a waiting list for community congregate living, arranging intake for home care services for residents returning home, assisting with transfer arrangements to other facilities, working in cooperation with outside agencies toward safe discharges);
  • Provides or arranges provision of needed counseling services and psychiatric medication management as indicated.

Qualifications

Education and/or Experience:

  • Minimum of Bachelor’s Degree in Social Work.
  • One year supervised social work experience working directly in a health care setting is preferred.

 

Certificates, Licenses, Registrations:

  • Current Social Work license in the state.  If working under a limited license a willingness and momentum toward full licensure with an agreeable timeline established by Administrator.
